Community Partners

CHHS: A Statewide Collaboration

CHHS is a collaboration between CPC Community Health and community-based organizations including local public health agencies, community clinics, and hospitals who host the CHWs.  In addition to providing local support to the CHWs, our  community partners help guide the implementation of CHHS in their communities.  This includes finding creative strategies for reaching the underserved, promoting the CHHS program and its services, and identifying and engaging local medical, healthy living and social service resources.
