CPC partners with Vuela for Health to design, implement and evaluate the program. Vuela for Health is a grassroots, promotores de salud/community health worker organization located in northwest Denver that strives to eliminate health disparities and break the cycle of poverty by leveraging the talents and abilities of community members. Their promotores de salud are passionate, talented and respected members of the community and strong advocates for health equity.
CPC, Vuela for Health, Northwest Colorado Health and High Plains Community Health Center rely on strong partnerships with over 50 health clinics, churches, schools, recreation centers and community organizations to implement CHARLAR in local communities
Northwest Colorado Health serves as CPC’s implementation partner in the Steamboat Springs and Craig areas of Routt County. Their mission is to improve quality of life for all Northwest Colorado residents by providing comprehensive health resources and creating an environment that supports community wellness.
CPC partners with High Plains Community Health Center’s WISEWOMAN program to implement CHARLAR in Prowers County. High Plains Community Health Center’s mission is to serve as the provider of choice for quality, accessible primary health care services in Southeast Colorado.
